Please take a few minutes to fill out this quick form to update the training staff on your current living, working, and social situation. The more we know about your daily routines and environment, the better we can match you with a puppy whose temperament will set both of you up for success!

If you are retired or do not work, please include details about any social activities, clubs or other regular activities you participate in.

If you are a student, please include details such as what program you are in/what are you studying, how many hours per week you are in class, the location of where you attend school, ect.

What type of activities do you enjoy doing in your down time?

Do you have any other dogs in the home? If yes, then please describe their breed, age and a small description of their temperament.

What mode of transportation do you predominantly use?

If raising a PADS puppy, it is expected that you will be available to attend puppy class regularly. Puppy classes are held:

Lower Mainland: Tuesday & Thursday evenings

Okanagan: Wednesday morning

Calgary: Thursday evening
